  1. Two Visa application forms. Congo Democratic Republic Visa Application for Citizens of United States of America
  2. Original, signed United States of America passport with at least 6 months of remaining validity.
  3. Passport-type photographs: 2
  4. Copy of round trip tickets or confirmed itinerary.
  5. Copy of International Certificate of Vaccination for Yellow Fever.
  6. A letter of invitation from the company or host to be visited in Congo Democratic Republic. If the applicant is visiting Kinshasa then the invitation must be notarized by the Ministry of Foreign Affairs. Applicants visiting other areas of Congo Democratic Republic can have their invitation notarized by a local city council or the MOFA.
