- Tourist Voucher (Invitation). Copy of tourist voucher (invitation) issued by tourist company or hotel in Azerbaijan approved by the Ministry of Tourism. Please upload a copy to your profile.
- If applicant does not have a Tourist Voucher, they must provide a copy of their hotel confirmation (or letter from travel agent) and a round-trip or onward flight itinerary

- Itinerary. Copy of round trip tickets or itinerary (Applicant’s name must be on itinerary)
- Official Letter of Invitation. An official invitation provided by the host, obtained from the Consular Department of the Ministry of Foreign Affairs of Azerbaijan or letter of invitation from the host along with a copy of the charter and state registration certificate of the host company/organization. A multiple entry visa will only be issued if the official invitation is issued by the Ministry of Foreign Affairs (MoFA) and specifies multiple entries. Confirmation of the invitation must be received directly by the Embassy prior to visa issuance.
