New U.S. Passport (Minor)

Required Documents Checklist:

  • Original or state certified copy of a U.S. birth certificate or Original naturalization certificate.
  • Two signed S. Passport application (DS 11) completed online and bar-coded.
  • Two passport photos taken within the past 6 months, 2×2 inches, and not from a vending machine.
  • Parental Consent Form
  • Please download, sign and print 2 copies of the Letter of Authorization. Include 1 copy in the sealed envelope and send the 2nd copy to the office outside the sealed envelope
  • Important: Parents need to sign the letter on behalf of the child. Please enclose an additional copy of the Letter of Authorization outside of the envelope sealed by the acceptance agent
  • Proof of Relationship to the minor applicant
  • Original U.S. birth certificate listing parents’ names; or Certified
  • Foreign Birth Certificate listing parents’ names and translation; or Consular Report of Birth Abroad (Form FS240) with parents’ names listed; or Certification of Birth Abroad (Form DS1350) with parents’ names listed; or Adoption Decree listing adopting parents’ names; or Court
  • Order Establishing Custody or Guardianship.
  • Proof of Identity for parent(s) or guardian(s)
  • A copy of a valid driver’s license or state issued I.D, issued more than 6 months ago, or U.S passport (mutilated, altered, or damaged passports are not acceptable as proof of identity), or Government or military ID (must be current).
    Note: Copies of both parents’ IDs must be included with the documents. If you have changed your legal name you will need to provide a Certified Copy of either a Marriage Certificate or a Name Change Court Decree.
  • Copy of the flight itinerary from a travel agent/airlines or an original letter with an original signature, from the employer describing the proposed trip, if traveling on business.

Required Process Checklist:

  • Take all of the documents listed above to a nearby US Post Office that is a passport acceptance agent or a County Clerk of Court’s office (appointments are required).
  • After presenting these documents, when requested, sign the application and attest to the truthfulness of the information on the application and present current government issued ID (e.g. driver’s license).
  • The acceptance agent will collect, separately, a $25 processing fee, and then the US Government fee. These fees are listed in the chart below as a total amount in the section “Government Fee”.
  • The acceptance agent will then seal everything into an envelope for security purposes. No one can open this envelope except the Department of State. Once sealed, it must be presented to the Department of State within 5 business days. The agent will return the sealed envelope to the applicant so that it may be “hand carried” by Matisse Services.

Final Instructions Checklist:

  • Place the sealed envelope, this work order form and any other notes or instructions and send to Matisse Services.
